Recently, Chen Gong, deputy director of the Department of Colorectal Medicine of the Affiliated Tumor Hospital of Sun Yat-sen University, and Wu Bin, deputy researcher of the Department of Pharmacy, Renji Hospital, Shanghai Jiao Tong University School of Medicine, were invited to Xinhua.com to introduce the treatment and economics of bowel cancer. Chen Gong pointed out that targeted drug therapy is a very important treatment for advanced intestinal cancer. However, due to economic factors, it has hindered the "steps" used by most patients in China.

"How to use these effective drugs on patients who need them most, this requires the joint efforts of the state, research institutions, clinical medicine, enterprises and other parties to expand the accessibility of drugs and make the allocation of health resources more cost-effective. Wu Bin said.

Bad living habits can easily cause bowel cancer

In recent years, malignant tumors have become one of the important causes of death in our country, including intestinal cancer. "In fact, bowel cancer is a kind of 'lifestyle disease', which has a lot to do with our usual lifestyle." Chen Gong said that as the saying goes, "the disease is from the mouth", some bad habits, such as high fat intake. The high protein content of the food is relatively high, the amount of cellulose intake is relatively small, or less exercise, may induce intestinal cancer.

What are the symptoms of bowel cancer? According to Wu Bin, for early colorectal cancer, common symptoms are changes in bowel habits. "For example, some people are used to defecation in the morning, but after the illness, they only defecate at night, not even defecation, and some people have changed the shape of the stool, which may be a sign of early bowel cancer." For patients with advanced colorectal cancer, symptoms such as weight loss and bloody stools often appear.

"In the face of some symptoms, we must not become a 'bird of surprise". To treat the problem scientifically and objectively, we must go to the hospital as soon as possible to seek medical treatment, and a qualitative examination by a doctor." Chen Gong also stressed that for asymptomatic people, Pay attention to bowel cancer screening, especially in the 45 to 50 years old. During this period, the first screening is required. According to the screening results, the doctor will set the frequency of the next screening. If the first screening is not available. The problem, and no family history, usually only 5 to 10 years before the second screening.

Advance multi-party payment mode to reduce the burden on patients with intestinal cancer

It is understood that cancer can be divided into 1 to 4, generally 1 to 3 is early cancer, and 4 is advanced cancer. "For advanced bowel cancer, check the extent of disease progression, including the location of cancer metastasis, and determine whether the metastasis can be removed. If it can be removed, we should cooperate with active chemotherapy and targeted therapy to restore the patient's health. If it can not be removed, you can use methods such as targeted therapy to control the patient's symptoms and prolong the patient's life." Chen Gong stressed.

However, for patients in the general family, it is often unable to afford the economic burden of using targeted drug therapy. For example, Chen Gong said that in China, patients who use colon cancer-targeted drugs need to pay up to 70,000 yuan per month. “The survey shows that the proportion of patients who can actually use targeted drugs in China is only 7%, while the other 90% do not use targeted drugs because of economic conditions.”

"At present, some entero-targeted drugs have been included in medical insurance in some provinces and cities in China, but they have not been included in the scope of medical insurance in most areas of China." Chen Gong pointed out.

How to make more patients get effective treatment and reduce the burden on patients? Wu Bin said that it is necessary to screen out patients who are effective in using targeted drugs and use health resources more efficiently in these patients. This is a very important measure to improve the economics of drug use. Second, the cost of new drug research and development itself is relatively high, and reforms in payment methods are needed. "According to foreign experience, a multi-party payment model can be adopted, which is paid by enterprises, medical insurance, and patients. For some patients who are particularly poor, if they meet the medication conditions, they can take the form of charity assistance."

Calculate the book of pharmacoeconomics and optimize the allocation of health resources

“Every patient is calculating the cost of treatment and effectiveness. For the country, how to make health resources work? Is it necessary to include this drug in medical insurance? Also need to consider medical expenses and changes. The most important factor in the economics of pharmacology is the cost performance, which can provide data reference to the country from a large scale." Chen Gong said.

It is understood that Chen Gong and Wu Bin conducted a pharmacoeconomic study on two colon cancer targeted drugs currently available in China through a sample survey. The study used the Incremental Cost Effectiveness Ratio (ICER) as an indicator. According to the World Health Organization (WHO), when ICER <1 times GDP per capita, it means that the cost of the new treatment is increased; when ICER<3 When the GDP per capita is increased, the added cost is acceptable; when ICER>3 times the GDP per capita, the increased cost is not worthwhile. Chen Gong said, "If you can extend a quality-adjusted life year, the added cost does not exceed three times the per capita GDP, which indicates that the new treatment method is cost-effective and worth considering."

The study found that certain drugs have economic value, and the cost-effective advantage is also obvious. Chen Gong pointed out that "this study is a very meaningful exploration for Chinese scholars to use their own data to find some treatments that truly meet international clinical requirements. Although this is only a sample survey in a local area, it does not represent the whole country, but Can provide direction for more extensive research in the future." (Miya Xiaoqian)
